How do I prove lost time from work?

If your employer pays you through a payroll check, you need to obtain documentation on the amount of time you were unable to work and the amount of money you lost. Including the number of hours you normally work, your rate of pay, etc. If you are self-employed, you generally need to provide copies of your income tax documents to substantiate your income. Also, you generally must have your doctor provide written documentation indicating time out of work to receive payment for your lost wages.
